Incident Investigations

At OEHS, we are committed to creating a safe and healthy environment for everyone at Wayne State University. By thoroughly investigating incidents and identifying their root causes, we can implement meaningful changes to prevent future incidents and continuously improve our safety performance.

Investigation Process

Our incident investigation process begins with an immediate response to establish safe working conditions and implement temporary safeguards. We then conduct on-site examinations, gather eyewitness accounts, and use root cause analysis to understand the circumstances surrounding each incident. Corrective actions are developed using the hierarchy of controls, prioritizing hazard elimination at the source. We use a risk-based approach to ensure that corrective actions address the most significant potential risks.

Employee Role

Employees play a vital role in incident investigations. Their firsthand accounts and knowledge of work processes are essential for understanding what happened and preventing future incidents. Employees may be asked to provide witness statements, participate in interviews, and contribute to the development of corrective actions.

Supervisor / Manager Role

During the investigation itself, supervisors/managers provide crucial insights into work practices, procedures, and training, identify witnesses, participate in interviews, assist with data collection, and help reconstruct the event. Post-investigation, they are responsible for implementing corrective actions within their teams, communicating findings to employees, monitoring the effectiveness of those actions, and reinforcing safe work practices to prevent future incidents. Their objectivity, cooperation, and support for employees involved are essential throughout the process.
